ACUTE KIDNEY INJURY IN PATIENTS WITH MYOCARDIAL INFARCTION AND NEW CORONAVIRUS INFECTION (COVID-19)

Acute kidney injury is a common pathological condition that complicates various diseases and affects both the short-term and long-term prognosis. There is ample research which testifies the unfavorable prognostic role of acute kidney injury in patients with myocardial infarction. The problem of acute kidney injury has become especially relevant during the COVID-19 pandemic, periodic outbreaks of which associated with new virus mutation are still observed. COVID-19 can both destabilize the existing pathology and lead to the development of new diseases, including myocardial infarction. In patients with myocardial infarction and COVID-19, a multiplicative risk effect of getting acute kidney injury is expected, the mechanism of which is likely to be different in each particular case: COVID-19 can simultaneously lead to a combination of renal and cardiac complications; acute kidney injury can develop due to heart failure, direct heart injury or myocardial infarction regardless of the new coronavirus infection. The article reviews the studies on the correlation between creatinine levels, renal dysfunction and the risk of myocardial infarction in COVID-19 patients. The authors describe the pathophysiological basis of cardiorenal correlation in COVID-19, which is characterized by the complexity and uniqueness of changes.

ОСТРОЕ ПОВРЕЖДЕНИЕ ПОЧЕК У ПАЦИЕНТОВ С ИНФАРКТОМ МИОКАРДА В СОЧЕТАНИИ С НОВОЙ КОРОНАВИРУСНОЙ ИНФЕКЦИЕЙ (COVID-19)

Острое повреждение почек является частым патологическим состоянием, осложняющим течение различных заболеваний и влияющим как на краткосрочный, так и на отдаленный прогноз. У пациентов с инфарктом миокарда неблагоприятная прогностическая роль острого повреждения почек продемонстрирована в значительном числе исследований. Особую актуальность проблема острого повреждения почек приобрела в эпоху пандемии новой коронавирусной инфекции (COVID-19), периодические вспышки которой отмечаются до сих пор, что связано с очередной мутацией вируса. COVID-19 способен как дестабилизировать течение имеющейся патологии, так и привести к возникновению новых заболеваний, в том числе инфаркта миокарда. У пациентов с инфарктом миокарда в сочетании с новой коронавирусной инфекцией ожидается мультипликативный эффект риска развития острого повреждения почек, механизм которого в каждом конкретном случае, вероятно, отличается: COVID-19 может одновременно вести к сочетанию почечных и кардиальных осложнений; острое повреждение почек может развиваться вследствие сердечной недостаточности, прямого повреждения сердца или инфаркта миокарда независимо от новой коронавирусной инфекции. В статье рассмотрены результаты исследований, касающихся взаимосвязи уровня креатинина, почечной дисфункции и риска развития инфаркта миокарда при новой коронавирусной инфекции. Описана патофизиологическая основа кардиоренальных взаимоотношений при COVID-19, характеризующаяся сложностью и уникальностью изменений.
