СИСТЕМА ГЛУТАТИОНА У БОЛЬНЫХ РАКОМ ЯИЧНИКОВ НА ФОНЕ ХИМИОТЕРАПИИ ПО СХЕМЕ АР

Рак яичников (РЯ) относится к опухолям с высокой чувствительностью к химиотерапии (ХТ). Эффективность последней снижается в результате выведения цитостатика из клеток опухоли. Система глутатиона обеспечивает устойчивость клеток к соединениям платины и антрациклинам. В то же время существенным компонентом противоопухолевого действия этих цитостатиков является активация образования активных форм кислорода, приводящая к возникновению оксидативного стресса (ОС). Система глутатиона выступает как антиоксидантный механизм, блокирующий проканцерогенный эффект ОС.

Цель исследования – оценка системы глутатиона в плазме крови, эритроцитах и опухолевой ткани при неоадъювантной ХТ по схеме АР.

Материалы и методы. В исследование были включены женщины с верифицированным диагнозом РЯ, которые были разделены на две группы. Пациентки 1-й группы получали неоадъювантную ХТ, 2-й – адъювантную ХТ по схеме АР. В плазме и эритроцитах пациенток 1-й группы, а также в опухолевой и внешне не измененной тканях яичников, полученных интраоперационно в обеих группах, определяли активность глутатионтрансферазы (ГТ), глутатионпероксидазы (ГПО), глутатионредуктазы (ГР) и уровень глутатиона (GSH). Обработку результатов проводили с использованием пакета программ Statistica Windows.

Результаты. Проведение 2 курсов ХТ по схеме АР приводило к значимому снижению активности ГТ и ГР на фоне постоянного уровня GSH в опухолевой ткани по сравнению с внешне не измененной. При этом в эритроцитах на фоне ХТ значимо снижалась активность ГТ. В плазме крови после ХТ по схеме АР значимо возрастало общее количество GSH.

Выводы. ХТ при распространенном РЯ, включающая цисплатин и доксорубицин, провоцирует в опухолевой и внешне не измененной тканях яичника ослабление системы глутатиона, способное спровоцировать сублетальный оксидативный стресс.

GLUTATHIONE TREATMENT IN PATIENTS WITH OVARIAN CANCER DURING AP COMBINATION CHEMOTHERAPY

Ovarian cancer (OC) is highly sensitive to chemotherapy (CT). The chemotherapy effectiveness is reduced as a result of cytostatic clearance from tumor cells. The glutathione system provides cell resistance to platinum and anthracyclines. At the same time, the activation of reactive oxygen, leading to the onset of oxidative stress (OS), is an important component of the cytostatics antitumor effect. The glutathione system acts as an antioxidant mechanism blocking the OS pro-carcinogenic effect.

The purpose of the study is to evaluate the glutathione system in blood plasma, erythrocytes and tumor tissue during neoadjuvant AP combination CT.

Materials and Methods. Women with a verified OC were enrolled in the study. The patients were divided into two groups. Patients of the 1st group underwent neoadjuvant chemotherapy, the patients of the 2nd group received adjuvant AP combination chemotherapy. The activity of glutathione S-transferase (GST), glutathione peroxidase (GPx), glutathione reductase (GR) and glutathione (GSH) level were determined in plasma and erythrocytes of Group 1 patients, as well as in tumor and externally unchanged ovarian tissues obtained intraoperatively in both groups. Statistica Windows software package was used to process the results.

Results. Two courses of AP combination chemotherapy led to a significant decrease in the activity of GST and GPx with constant level of GSH in the tumor tissue. At the same time, GST activity significantly decreased in erythrocytes during CT. The total amount of GSH in the blood plasma significantly increased after AP combination CT.

Conclusion. CT (cisplatin and doxorubicin) in patients with OC leads to weakening of the glutathione system in tumor and externally unchanged ovarian tissues. This process can cause a sublethal oxidative stress.
