CHRONIC KIDNEY DISEASE IN PATIENTS WITH CHRONIC LYMPHATIC LEUKEMIA AND THEIR SURVIVABILITY

Chronic lymphocytic leukemia (CLL) is one of the most common lymphoproliferative diseases in the European population with an increase in the incidence in the elderly and senile age. However, it is among the elderly that a decrease in glomerular filtration rate (GFR) and concomitant chronic kidney disease (CKD) are associated with the severity in long-term prognosis.

The aim of the study was to analyze CKD incidence and prognostic value in patients with CLL.

Materials and Methods. CLL retrospective analysis was performed in 132 patients (60 men and 72 women). CKD was diagnosed according to the 2021-Guidelines of Russian Scientific Society of Nephrologists. Results. Among the examined patients, 64 (48.5 %) were diagnosed with CKD: stage C2 – in 23 patients. (17.4 %), stage C3a – in 41 patients. (31.1 %). CKD incidence in patients with CLL was higher than in the similar population without CLL. The authors revealed that there was no correlation between CLL stage and CKD severity. Survival analysis showed that only 43 patients (32.5 %) overcame the estimated Binet median survival. C3a in patients with CLL at the time of CKD diagnosis is strongly correlated with survival.

Conclusion. CKD occurs in 48.5 % of patients with CLL. It has been established that C3a CKD worsens CLL patient survivability.

ХРОНИЧЕСКАЯ БОЛЕЗНЬ ПОЧЕК У ПАЦИЕНТОВ С ХРОНИЧЕСКИМ ЛИМФОЛЕЙКОЗОМ И ИХ ВЫЖИВАЕМОСТЬ

Хронический лимфолейкоз (ХЛЛ) – одно из наиболее распространенных лимфопролиферативных заболеваний европейской популяции с увеличением частоты встречаемости в пожилом и старческом возрасте. Однако именно среди пожилых снижение скорости клубочковой фильтрации (СКФ) и наличие хронической болезни почек (ХБП) ассоциировано с наибольшим ухудшением долгосрочного прогноза. 

Цель исследования – проанализировать встречаемость и прогностическое значение ХБП у пациентов с ХЛЛ.

Материалы и методы. Был проведен ретроспективный анализ течения ХЛЛ у 132 пациентов (60 мужчин и 72 женщины). Хроническую болезнь почек диагностировали, основываясь на Рекомендациях НОНР, 2021.

Результаты. Среди исследованных пациентов у 64 (48,5 %) была обнаружена ХБП: стадия С2 –у 23 чел. (17,4 %), стадия С3а – у 41 чел. (31,1 %). Частота встречаемости ХБП у пациентовбс ХЛЛ выше, чем в аналогичной популяции без ХЛЛ. Обнаружен факт отсутствия связи между стадией ХЛЛ и выраженностью ХБП. Анализ выживаемости показал, что только 43 пациента (32,5 %) преодолели расчетную медиану выживаемости по классификации Binet. Наличие у пациентов с ХЛЛ на момент постановки диагноза ХБП С3а находится в сильной прямой связи с выживаемостью.

Выводы. Хроническая болезнь почек наблюдается у 48,5 % пациентов с ХЛЛ. Установлено, что ХБП С3а ухудшает выживаемость больных с ХЛЛ.
