Age-related fertility decline is associated with oocyte oxidative damage and circadian dysregulation. Melatonin acts as an endogenous antioxidant and a potentially modifiable factor in IVF failure.
Objective. The study aimed to evaluate the prognostic value of melatonin deficiency and the efficacy of personalized circadian disorder correction taking into account MTNR1B polymorphisms.
Materials and Methods. This prospective study enrolled 239 women (aged 35–45) with idiopathic infertility and previous IVF failures. The study group comprised patients with salivary melatonin deficiency (<40 pg/mL; n=115), while the comparison group included women with a normal circadian profile (n=124). Patients in the study group underwent 2–3 months of preconception counseling and care with agomelatine and melatonin. The authors determined the diurnal melatonin profile, follicular fluid melatonin levels, estradiol-to-testosterone (E2/T) ratio, endometrial expression of LIF and CD56, and the MTNR1B rs1387153 polymorphism. Additionally, serum levels of insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) were evaluated. The IGF-1/IGFBP-3 ratio was calculated.
Results. Low nocturnal melatonin levels were associated with increased follicular fluid testosterone concentrations (ρ=-0.52; p=0.005) and decreased LIF expression (ρ=0.45; p=0.016). The patients without the mutation showed normalization of melatonin concentration. The fertilization rate (74.0 %) and clinical pregnancy rate (29.8 %) did not significantly differ from the comparison group (p>0.05). Conversely, T-allele carriers demonstrated a limited response, with a clinical pregnancy rate of 8.9 %. A nocturnal melatonin level below 31 pg/mL predicted pregnancy failure (AUC = 0.91). The IGF-1/IGFBP-3 ratio was significantly higher in the study group compared to the comparison group: 42.7 [37.6–49.4] a.u. versus 40.1 [31.7–47.4] a.u., respectively (p=0.0178).
Conclusion. Melatonin deficiency represents a modifiable risk factor for IVF failure. Evaluation of the circadian MT profile and MTNR1B genotyping enables patient stratification and the selection of personalized preconception correction strategies.
Key words: melatonin, age-associated infertility, circadian rhythms, IVF, ovarian reserve, endometrial receptivity, MTNR1B polymorphism, personalized therapy.
